What is a Case Manager?

Case managers are trained professionals who provide services designed to support individuals and families with addressing barriers, identify strengths, and collaboratively develop plans to improve the quality of situations a person may otherwise find challenging.

Case Managers can assist with:

  • Collaborating and streamlining multidisciplinary treatment services

    • (i.e., helping integrate the treatment plans of your psychiatrist, therapist and addictions counselor)

  • Crisis assistance planning

  • Advocacy for the client and family

  • Assistance with navigating community and state resources

    • Supplemental Nutrition Assistance Program (SNAP)

    • Kentucky Temporary Assistance Program (KTAP)

    • Medicaid Insurance

    • Child Care Assistance Program (CCAP)
